Breast Pump Orders:

When ordering a breast pump for mom via ccLink, a pump request should also go to WIC. In outpatient setting, do this via ccLink - PHN Lactation referral. Inpatient a form should be completed by LC and faxed to WIC.

  • CCHP/Medi-Cal HOSPITAL GRADE BREAST PUMP Order first on mother's chart via ccLink (Inpatient DME Hospital Grade E0604 double electric - marked URGENT) AND fax order form to ITC Medical

  • For NON CCHP BREAST PUMPS - BOTH HOSPITAL GRADE AND BACK TO WORK: enter PHC Lactation request and specify URGENT need for breast pump. Also, if inpatient, work with Lactation Consultants to expedite this. Referrals may also be made by Care Coordinator using this Hygeia form:

  • CCHP BACK TO WORK BREAST PUMP - DME double electric E0603. Enter the order on mother's chart via ccLink specifying Vendor as Hygeia or Pumping Essentials. Also specify "RETURNING TO WORK/SCHOOL"


  • WIC Pump Loan Program: Contact Araceli (Shelly) Trejo at 925-646-5534.

  • In outpatient clinic send WIC pump request via ccLink - PHN Lactation. For inpatient, complete and fax the following form: .
